Closing the Gap focuses on the creation and training of community health workers (CHWs) to help bridge the gap between clinical and community settings. CHWs are “non-clinical workers who come from the communities of the patients that they serve and whose job is to help those patients be healthier within the context of their lives as well as to help providers better understand and respond to patient needs.” Looking at global and domestic CHW training programs, Closing the Gap provides a framework for developing sustainable programming in local communities around the US.
