Population Health: The Ghost Aim

Population health is cited as the “ghost” of the Triple Aim in healthcare, noting that “only 10% of health outcomes are affected by medical care, whereas 60% are rooted in social and environmental factors and associated behaviors — which are the essence of population health.” Uncertain or ambiguous definitions of population health make it a difficult objective to achieve, but the article calls out that it should be the first thing defined out of the Triple Aim - with improved care and reduced costs resulting from better population health efforts.
