The Sussex County Health Coalition, in partnership with the Delaware Center for Health InnovationsHealthy Neighborhoods Council, is proud to announce that the partnering members of the Healthier Sussex County Task Force have been chosen to receive the 2017 Sussex County Health Innovation Award.   The Healthier Sussex County Task Force is a collaborative effort of community members lead by the CEO’s of each Healthcare System in Sussex County. Those Healthcare Systems include, Beebe Healthcare, Nanticoke Health Services, Bayhealth-Milford and La Red Health Center.

The Healthy Neighborhoods Local Council will honor contributions made by each Healthcare System by recognizing the organizations’ work, as a part of the Healthier Sussex County Task Force. Each have had a major impact on the emerging landscape of Healthcare transformation in Sussex County, and are viewed as a “Best Practice”, now being replicated statewide.

Each team will be invited to attend the Sussex County Health Coalition Quarterly Meeting for the Inaugural 2017 Sussex County Health Innovation Award Presentation, scheduled for July 20th from 8:30 am to 10:30 am.   This meeting will be held at the University of Delaware Cooperative Ext., 16483 County Seat Highway, Route 9, Georgetown, Delaware. It is free and open to the public.

The vision and work of DCHI are grounded in the Triple Aim of improving health, health care and the patient experience, and reducing the growth in per capital health care cost in Delaware, plus one ─ improving the provider experience.

As the lead agency for the roll out of the first Healthy Neighborhood in Sussex County, The Sussex County Health Coalition is focused on building a “Culture of Health” for all our citizens. Through our partnership with Healthier Sussex County Task Force and DCHI, we have been able to develop an action plan with a set of tangible measurements to help our entire county. This community plan is a collective impact framework using the “Social Ecological Theory of Change” to foster this work, in creating a culture of health. Sussex County’s Culture of Health is broken into four interconnected action areas that we are aligning with the current community strategies.

These action areas are:

  • Making heath a shared value
  • Fostering cross-sector collaboration to improve wellbeing
  • Creating healthier, more equitable communities
  • Strengthen integration of health services and systems

The Sussex County Health Coalition exists to engage the entire community in collaborative, family-focused efforts to improve the health of children, youth and families in Sussex County. Together, we can make Sussex County the leading Healthy Community in Delaware and a driving force for Health Care Transformation locally and statewide. I know we will face challenges but together we can innovate and attain what we envision for ourselves, each other and our community.
