This study/report looks at 15 successful ACOs who have clearly defined patient centered care and designed a three pronged approach towards the practice: 1) patients as partners, 2) proactive customer-service orientation, and 3) care coordination with a whole-person approach. This article discusses how having a clear definition and universal approach to patient-centered care can provide a framework for moving from “volume to value” and allow organizations to make decisions about managing care based on their unique structure.
