CHNA Subcommittees
The CHNA Work groups will oversee the integration of clinical data and the coalescing of common metrics across the clinical delivery system and with community-based organizations for the purposes of aligning strategic initiatives to addresss converging clinical needs and social determinants of health.

Data Subcommittee
The Data Committee helps to support local Healthy Neighborhood Councils with a focused, consistent and viable plan for data collection and management, and to help gauge and measure the success of planned strategies for improving population health.
Sustainability Subcommittee
The HN Sustainability Committee works to provide recommendations and solutions for sustainability of the Healthy Neighborhoods programs that are established across the state, working to align and unify potential funding streams in targeted neighborhoods, bringing about a more holistic approach, and sharing resources to address population health issues.
