A brief article  highlights how “hospitals caring for a high proportion of high-needs patients provided better care overall.” The five key elements to successful care for high needs patients include understanding the diversity and unique situation of each patient, coordinating care such as behavioral health integration and enhanced primary care, and engaging more with patients and families.
