2017 Community Health Workers Report Download

In association with the Delaware Department of Health and Social Services (DHSS) and Health Management Associates (HMA), we have released the Community Health Workers (CHW) official report to the public.

In this report we have outlined strategies for development and deployment of community health workers in the state of Delaware. A plan is devised throughout the report regarding the importance of education, certification and training of these workers.

The report includes five key recommendations and one for further consideration:

  1. Establish a Community Health Worker Certification Program – that includes approved training to meet established CHW competencies   
  2. Establish a Community Health Worker Certification Board – to oversee the development and maintenance of the certification program, free of unnecessary barriers
  3. Establish a Curriculum Development Committee – to create the curriculum that will form the basis of the training program
  4. Establish Two Entry Points for Training – one for high school students, and another for current CHWs seeking certification and for aspiring CHWs who are not high school students
  5. Develop Financing Mechanisms – examining Medicaid, hospital community benefit activities, population-based payment models, and more
  6. Further Considerations – including using “hot spotting” to identify high-need neighborhoods, and encouraging organizations that employ CHWs to collect common metrics
